OK, it might be a cruel practical joke, but it’s still nice to see poor Dilbert gettin’ a little action. Pretty risque’ knee placement for the funnies page, eh?!

I didn’t get this, because wouldn’t the joke be on the doctor? She has to feel up an engineer, for cryin’ out loud! How is that to her advantage? Looks like Dilbert wins here, even if he thinks he’s dreaming it, so where’s the practical joke?

AMC - Dilbert’s tie is always curled up like that. In fact years ago Dilbert had a girlfriend named Liz and everyone was speculating if Dilbert was going to get lucky.
Scott Adams (Creator and model for Dilbert) publicly announced that if and when Dilbert got lucky his tie would be straight and flat.
A few weeks/months later it was flat.

I’ve actually felt I should be offended by Dilbert’s tie. Now I know I should.

Nope she was blond and had a ponytail. She met Dilbert at a soccer game where she rebounded a ball off his head to score a goal.
There is some ambiguity as to whether they actually did it.

As I understand it, there is little ambiguity. The strip right after the hypothetical “deed” showed Dilbert with his usually upturned tie lying completely flat, almost as if a great tension had been released, in a strip directly referring to Dilbert’s love life.
I think the “veterinarian” line is a joke about Jon Arbuckle’s current girlfriend Liz.

The ambiguity lies in the dialog for that particular strip. Dogbert poses a question about Dilbert’s religion (if I recall correctly, he notices that Dilbert has been acting strangely and comments that Dilbert either had sex with Liz or suddenly found religion), to which Dilbert answers “I think I’m a Unitarian”.
The “Unitarian” part provides the ambiguity, as the “uni”, coupled with the question of whether Dilbert had sex with Liz, creates the notion that maybe it was a sexual encounter involving only one person.
I do not recall if Scott Adams ever confirmed it one way or another.
