Okay, going off the name of the company (”In Utero Aesthetics, Inc.”), it sounds like the couple recognizes that they’re…well, not particularly attractive (note her buck teeth and chin whiskers, and his baldness, sagging jowls, and coke-bottle glasses). The implication is that they would prefer that their child not be cursed with their looks, and they’d rather have genetic manipulation done (note also the DNA spiral on the sign) to make their offspring more attractive. The doctor (specialist, whatever) is warning them that the gulf between one generation and the next–the generation gap–simply gets worse if the child is also more attractive than its parents, as well. (”You’re just to old to get it. And you were never popular anyway!”)
The place is called “In Utero Aesthetics Inc”; they presumably use genetic engineering to make your offspring more attractive. The customers appear to be rather unattractive (balding, buckteeth, etc). The doctor is warning them that not only will they have trouble relating to their children because they are of a different generation, but they will also have trouble relating to them because they will be much more attractive than their parents.
I’m guessing that because the couple is at “In Utero Aesthetics, Inc.” that they are trying to have an attractive child. From the looks of them from behind - buck teeth, slack jaw, hairy ears, thick glasses and a chin hairs on the lady - I’m supposing that these two would naturally have a pretty homely child. So if they use this company to have an attractive one they are going to have a gap in age and in looks. That’s gonna be one ugly bomb that’s gonna hit in a few years.
They remind me of Harvey Pekar’s portraits of his self and wife in in the great comic and movie American Splendor. I think Lovely considers the Pekars fucking ugly.

Rather subtle, but it is Robert Lovely.
